Letters There is an additional charge of $25 for letters of medical necessity and disability paperwork, if not done in conjunction with a visit to the office. Other requests for paperwork and authorizations will be charged at the discretion of the physician, but generally most should be done at an office visit. Medication Refills Effective immediately, the doctors will no longer accept prescription refills by telephone request. Please contact your pharmacy for a refill request. Keep in mind, the doctors strive to provide refills until the next appointment. If your refills have run out, please schedule a follow-up appointment. Laboratory Work We are equipped to draw blood for most laboratory tests processed by Quest labs, and LabCorps. We also do urinalysis, pap smears, strep tests, and cultures as needed. Orders from other physicians offices can easily be done in our office with their written order, but an office visit co-pay, or venipucture fee will be assessed. Referrals Certain insurance companies require formal referral for specialist care. They must be completed in advance of the visit with the specialist through our referral coordinator. With the exception of true emergencies, we request a 5-day notification of the need for the referral. Please take note: we cannot do same-day referrals. Billing Medical insurance billing has become extraordinarily complex, and we will bill your insurance as a courtesy to you. We send statements monthly to you so that you may be aware of the status of your account, and whether insurance has paid their portion. Any remaining balances should be paid in full as soon as possible. If your insurance has delayed or refused payment, we encourage you to contact your insurance plan to remedy the situation as you will be responsible for any outstanding debt. If your account remains unaddressed and unpaid after two billing cycles, it will be turned over to a collection agency and you will no longer be able to schedule appointments with the doctors of this practice. Late fees and no-show fees may be applied to your balance if you do not make it to your appointment and you fail to call and cancel or reschedule 24 hours in advance. Fasting Information If you are coming in for a morning appointment and are having fasting labs, do not eat anything after midnight the night before the exam. The morning of the exam you may have a cup of BLACK coffee or PLAIN tea. We encourage you to stay hydrated by drinking plenty of water. If you are coming in for an appointment during the afternoon hours (after 1pm) you may eat a light, low-fat breakfast no later than 7am. (plain toast, piece of fruit, cereal, oatmeal and beverages listed above). Between 7am and your appointment time you may have an additional cup of black coffee or tea, and are encouraged to drink plenty of water.
